Age Group Specifications:
-14U – born on or after 7/1/94
-19U – born on or after 7/1/90
Checkin Credentials:
1. First, Signed, stamped team membership application
2. Second, review stamped and signed team roster
3. Third, match birth certificates for each player on roster
4. Forth, match USA Hockey Waiver form for each player on roster
5. Fifth, match USA Consent to Treat forms for each player on roster
