Age Group Specifications:
-13U State – born on or after 7/1/97
-High School 14U/19U – born on or after 7/1/92
Checkin Credentials:
1. First, review stamped and signed team roster
(Teams should have a Stamp by their Registar replacing the Birth Certificates verification.)
2. Second, match USA Hockey Waiver form for each player on roster
(Waiver is not needed if USA Roster has the check mark signifying completion on-online.)
3. Third, match USA Consent to Treat forms for each player on roster
